PAD-T talus approach

The dashed red line demonstrates one avaliable approach to the talus for the PAD-T. Palpate the floor of the sinus tarsi and make your stab incision at this level. Advance the stieman pin to the lateral neck of the talus and proceed with the bone cortex incision angled tword the talus body. This is a preferred approach when the bone infection is primarily in the body of the talus.
When the bone infection is located within the talar neck or head a dorsal approach may be utilized with intra-operative fluoroscopy. This is illustrated in the images below.

This case demonstrates a dorsal talus bone cortex incision approach. This case presented with calcaneal and talar osteomyelitis, status post partial calcaneal resection by a different surgeon. The remaining calcaneus had the
PAD-T completed first before the talar PAD-T was done.
