In Preparation of your visit/consultation
Please note: Dr. Karr treats the lower leg and foot (below the knee only)

The information checklist listed at the bottom of this page "Patient Visit Checklist" needs to be sent to me before my office can schedule a phone consultation.
Please do not call the office for an appointment if you have not read this page.

OVERVIEW OF PROCESS:
1st - Download and print the form below so you can send the required information via mail.
2nd - Once received, the office will then schedule a consultation with you and the doctor to take place over the phone (there is a charge for reviewing your information and discussing next steps with you.)
3rd - After the phone meeting, if you decide to move forward, an appointment will be set in the office and the procedure will be scheduled.

Once initial documents have been received, you may email follow-up needs. Please do not send links to your patient portal to find your information. You can email documents only, no images to: drkarrcoe@gmail.com. Records may also be faxed to 863-644-2847 if you call first and it is less than 10 pages.
2. I need your records to review before your phone consultation. I will review your records before I talk to you, and my staff may call you with questions. For patients that are traveling, after I have reviewed your records a telemedicine appointment will be made for you before your are given an office appointment. Generally speaking, I will try to schedule your surgery as soon as possible.
3. If you email me--pictures of films with a smart phone are usually fine. It is most helpful for me to review your MRI/CT/xray studies (the actual films). I cannot stress how important it is for me to review the films. You can generally ask the location that you had the films done at to make you a CD and have that CD mailed to me. Please mail these items to me. Once these items are received and reviewed you will be contacted.
4. Also send a list of your doctors you have seen with their phone numbers, fax numbers, and addresses, along with a medication list.
5. Please do not schedule an appointment without notifying as to the purpose. Osteo patients health records need to reviewed. It usually takes a significant amount of time to review this information. Not sending the above items before your visit will probably delay your care.
